The process.

With Home Choice MI, the process is simple.

Step 1.

Call or Apply Online

Reach out to us and tell us who needs help. We’ll do a quick pre-screen, answer your questions, and start the process immediately.

We do it all: paperwork, scheduling, and state requirements — nothing is left to you.

Step 2.

Eligibility Check

The person receiving care must be on Medicaid and need help with daily activities such as bathing, dressing, or mobility.
We confirm eligibility quickly and guide you every step of the way.

Step 3.

Choose Your Caregiver

Pick a caregiver you trust — a family member, friend, or a local professional.

Caregiver requirements:

* Must be 18 years or older
* Must pass a criminal history screen
* Must have a valid photo ID
* Must enroll in CHAMPS (state payment system)

Don’t worry — we handle the entire process for your caregiver. They just provide basic information, and we take it from there.

Step 4.

Paperwork — We Handle It

We complete and submit all required forms, including:

* DHS-390 (Application for Home Help Services)
* DHS-54A (Medical Needs Form)
* MSA-4676 / MSA-4678 (Provider Agreements for Caregivers)

You don’t chase forms or figure out CHAMPS. We prepare, collect, and submit everything.

Step 5.

In-Home Assessment

A state Adult Services Worker visits the home to confirm needs and create a Plan of Care (sometimes called “Time & Task”).

This Plan of Care decides how many hours of care are approved. Reviews happen every six months.
We schedule, prepare, and track this for you.

Step 6.

Authorization & Start of Care

Once the state approves both the client and caregiver:

* Care begins right away
* We train the caregiver on simple service verification (so hours get logged correctly)
* Payments start flowing through the state

Step 7.

Ongoing Support

We stay with you for the long run.

* We track assessments and renewals
* We keep CHAMPS active and in good standing
* We monitor hours and handle any changes

If anything comes up, you call us — we take care of it.

How Long Does It Take?

Day 0–2

We complete pre-screening and submit all paperwork.

Week 1–3

Home visit and assessment are scheduled; caregiver file submitted to CHAMPS.

By Day 45

State issues approval and Plan of Care.

Immediately after approval:

Care begins and caregiver starts getting paid!

Many families are up and running within just a few weeks.
